Bottle Preps
AmTest will provide all the necessary sample containers for your project at no additional charge. This includes only the containers that the laboratory requires for testing. Additional containers (i.e. archived samples, samples submitted to other laboratories, etc.) are the responsibility of the client. Arrangements for the proper container must be made at least five working days in advance.

Sample Receipt
Samples must be received in proper containers and with sufficient volume to perform all of the required analyses. Samples to be archived will not be accepted until such time that they are to be analyzed. Samples received after 4 p.m. will be logged in to our system the following business day. It is in the client's best interest to COMPLETELY fill out proper chain of Custody Forms or submit a detailed letter of transmittal at the time of sample delivery. It is the CLIENT's responsibility to ensure that all the information (sample ID's, dates, specific anlyses, etc.) is correct and specific to each sample. Please be specific with respect to the destination of the final report as well.
  • All microbiology testing must be received before 3:00 p.m. to be analyzed that day, otherwise the samples will be anlyzed the following morning.
  • All microbiology samples must be received before 2:00 p.m. on Fridays and/or prior to a holiday.
Quality Control Deliverables
All available quality control will be provided with the analytical results. Any modifications to the standard quality control reporting is considered billable samples. Chromatograms, mass spectra, bench sheets, computer printouts, etc. are the property of the laboratory. Arrangements for the submission of any raw data must be made at the time of the price quote. Failure to do so will result in additional charges.

Sample Turnaround
Estimations (based on CURRENT WORKLOAD) of when the client may expect results are made at the time of initial contact with AmTest project manager. Turnaround times are reviewed and modified on a weekly basis, beware that turnaround commitments cannot be guaranteed if samples are not submitted in the original time frame specified in the initial agreement.

Revised Reports
Any requests for revisions of the laboratory reports must be made within 30 days of receipt. Changes after this period will be billed at an hourly rate of $60.00 per hour (one hour minimum). All reports are computer generated. Customization of report format will be at the expense of the client.

Sample Storage
Samples are stored for 60 days beyond the submission of the analytical report. Modifications will require advanced notification and a fee may be charged. Although we have the ability to archive samples, space is limited. Arrangements for space, storage time and price will be negotiated. Classified wastes will be returned to the client.

Price Quotes
Price quotes are project specific and are effective for 30 days. Submission of samples after the 30 day period may be subject to price increases.

Invoices
Invoices will be submitted with each analytical report. Terms are net 30. Second party invoicing is prohibited without written consent of AmTest, INC.

Rush Service and Charges
Rush service is on an available basis only. Please contact the laboratory in advance of any rush analysis requests. Rush analyses may incur a percent surcharge of the standard price and usually follows the following breakdown:
  • Same day = 200%
  • 24 hour = 100%
  • 2 day = 75%
  • 3-4 day = 50%
  • 5-7 day = 25%
All rush analysis charges must be acknowledged prior to or concurrent with sample submittal. A standard form is available at the laboratory for this acknowledgement. Please note that rush commitments cannot be guaranteed if the samples are not submitted in the time frame that was originally agreed upon.

**LIABILITY
AmTest, INC. is not an insurer or a guarantor. AmTest, INC. represents that services shall be performed within the limits mutually agreed to, in writing, and in a manner consistent with the level of care and skill ordinarilly exercised by other providers of similar services under similar circumstances. Client understands and agrees that AmTest, INC. in entering into this contract and by performing services hereunder does not assume, abridge, abrogate or undertake to discharge any duty or responsibility of Client to any other party or parties. No one other than Client shall have any right to rely on any report. Client agrees, in consideration of AmTest, INC. undertaking to perform the test(s) or program hereunder, to protect, defend, indemnify, save harmless and exonerate AmTest, INC. from any and all claims, damages, including lost profits, expenses, including attorney's fees, either direct or consequence of the performance of the testing hereunder and/or the performance of the products tested hereunder. Any liability shall in no event exceed the amount of the fee paid by the Client for such services and Client's sole remedy at law or in equity shall be the right to recover up to such amount. Client acknowledges and agrees that in no event will AmTest, INC. be liable for consequential or incidental damages or expenses, including, but not limited to lost profits.
